Job Title: Pharmacist

Job Description:

1. Consults with patients regarding individual medications or supplies prescribed including dosage frequency specialized requirements and possible drug interactions.

2. Responsible for the evaluating preparing and dispensing medications.

3. Verifies prescribed medications according to professional standards.

4. Performs medication reconciliation processes to extend a seamless continuum of care for patients upon discharge.

5. Ensures and behaves with a focus on excellent customer service and clinical care.

6. Consults with patients regarding individual medications or supplies prescribed including dosage frequency specialized requirements and possible drug interactions.

7. Interacts closely with individual patients in order to answer all questions and/or concerns regarding their medication therapy.

8. Reviews and evaluates patient's plan of care for therapeutic appropriateness.

9. Assists in the fiscal management of the ambulatory pharmacy to optimize to optimize profits and control expenses for the pharmacy consistently consulting with the pharmacy manage.

10. Makes recommendations to the pharmacy manager regarding any changes to improve operating procedures or to improve and facilitate patient care.

Requirement:

1. Ability to communicate effectively both orally and in writing

4. Strong interpersonal skills

5. Ability to resolve patient issues quickly and creatively in order to improve customer satisfaction

6. Familiarity with pharmaceutical products services and drug interactions

8. Good analytical skills

9. Knowledge of Microsoft Word Excel PowerPoint and Outlook
