Job Position: Clinical Abstractor

Job Summary


  • We are seeking a meticulous and knowledgeable Clinical Abstractor to join our healthcare data team remotely.
  • In this role, you will review patient records, extract relevant clinical information, and ensure accurate documentation for reporting, research, and quality improvement purposes.
  • Your work will directly contribute to improving patient care, regulatory compliance, and health data integrity.

Responsibilities

  • Review patient medical records, charts, and clinical documentation for accuracy and completeness.
  • Extract, code, and summarize relevant clinical data according to established guidelines and standards.
  • Ensure proper use of coding systems such as ICD-10, CPT, and HCPCS.
  • Collaborate with healthcare providers and internal teams to clarify and verify information as needed.
  • Maintain patient confidentiality and comply with HIPAA and other regulatory standards.
  • Enter data accurately into electronic medical record systems or designated databases.
  • Participate in quality assurance audits and provide feedback to improve documentation practices.
  • Keep up-to-date with clinical guidelines, coding standards, and healthcare regulations.

Requirements

  • Bachelor’s Degree in Nursing, Health Information Management, Medical Coding, or related field.
  • Registered Nurse (RN) or other clinical background is highly advantageous.
  • 3+ years of previous experience as a Clinical Abstractor, Medical Coder, or in health information management.
  • Strong knowledge of medical terminology, clinical documentation, and coding standards.
  • Excellent attention to detail and data accuracy.
  • Proficiency with electronic medical records (EMR/HER) systems and Microsoft Office Suite.
  • Strong organizational and time-management skills.
  • Effective written and verbal communication skills for remote collaboration.
  • Ability to work independently in a remote environment while meeting deadlines.
  • Knowledge of coding systems and clinical documentation requirements.
  • Ethical handling of sensitive patient information.

Salary
N500,000 / month (Non-negotiable).
