• Certificate as Registered Nurse
• Certificate as Registered Midwife
• Certificate as B. Sc. Nursing
• Current Practicing License
• Local Government Identification
• Birth Certificate / Age Declaration [Not more than 45yrs]
• First School Leaving Certificate
• WAEC / NECO
• NYSC Certificate (Where applicable).
